In that Mercola-Dinkov interview, they did not discuss the estrogen pathways important for understanding estrogen's benefits and potential harmful effects.
If I was understanding the Dinkov interview correctly, Dinkov was saying that adding in exogenous estrogen after menopause is harmful. I think this comes down to the individual and their labs.
When your health professional runs a 24-urine hormone panel like the DUTCH test, it will show you a chart with the ratio between 2-hydroxyestrone and 16α-hydroxy-estrone. This ratio is vital to know to help determine your risk of breast cancer.
“Good” and “Bad” Estrogen Metabolites
2-hydroxyestrone is considered your “good” estrogen metabolite, and 16a-hydroxyestrone is regarded as the estrogen metabolite that increases breast cancer risk. However, you also do not want a low 16α-hydroxy-estrone level either, which plays a role in maintaining bone density.
In some cases, BHRT can be useful after looking at a detailed hormone panel, but not in every case. In some instances, other options other than BHRT can be beneficial.
What to do if Your Estrogen Ratio Indicates Breast Cancer Risk
For instance, a whole foods diet and having adequate cholesterol levels (not too low) can support hormonal health.
Other useful add-ins include but are not limited to milk thistle and liver support, DIM or indole-3-carbinol, calcium glutarate, schizandra, nettle leaf, green tea, sulfur-rich foods, beneficial herbs such as turmeric and rosemary.

You can also avoid processed body care products with added chemicals and fragrances to support optimal estrogen health. Making changes can be helpful if you consume the standard American processed food-like diet or are overweight. If you carry excess weight around your abdomen, this is hormonally active fat, which can increase your estrogen levels.
Elimination is also crucial since excess estrogen is removed via the bowels. If you are chronically constipated, the extra estrogen can recirculate into the body.
The bottom line is that knowing your estrogen ratio could be helpful and a motivator to make some diet and lifestyle changes to reduce breast cancer risk.
